If you use our "Contact Us" or Web Survey form we will respond to you via the method you have indicated unless you check that you do not want to be contacted. In either case, the information collected will be used solely for site and practice evaluation. We do not provide third-party access to any information obtained through our website.If you are under age 18, you must receive permission from your parent or guardian to use your name, e-mail address or any other identifiable information on any of our forms. Country Midwife, LLC complies with all local, state and federal regulations for protecting patient personal and medical information (our Privacy Policy is available upon request).
Complaints:
From time to time, family members, friends, outside parties, medical providers may have concerns, questions or complaints about something they heard about or experienced during someone's course of care with us. We encourage such concerns to be addressed openly with us, and we strive to resolve them to the satisfaction of the parties involved, in as much is possible without compromising client privacy. However if you are not the client themselves, you must present written permission of the client to discuss the matter with us.

If your concern cannot be resolved through our internal process, a written complaint can be filed with one or more of the following entities:
